MEDICARE REBATES
Chronic Disease Management Plans (CDM), formally called Enhanced Primary Care (EPC) Plans are a Government funded initiative that allows for Allied Health Professionals to provide Medicare services to help rehabilitate and manage people with chronic disease, illness, and/or injury. Exercise Physiologists are recognised under this scheme and are considered experts in the field of lifestyle management, exercise prescription and the implementation of positive health behaviours for a clinical population.
CDM/EPC Plans allow for people with chronic disease and/or injury to access professional services to manage their condition with the assistance of a Medicare Rebate. The goal of any CDM/EPC Plan is to enhance the health, clinical status and quality of life of the patient. An CDM/EPC Plan provides for up to five sessions with an Allied Health Professional within a calendar year and a rebate of $52.95 per session via Medicare.
